Cost

$15 Office Visit fee plus $10 administration fee per shot. Medicaid accepted for some services, please present card to nurse.
The Ohio Department of Health Sliding Fee Scale may apply.
Some services can not be denied due to inability to pay

Your child may not be eligible for some vaccines due to Vaccines for Children Program requirements. Ask your nurse.
