    The Monroe County Health Department Vital Statistics offers Birth and Death records from 1909 to date.  A birth or death must have occurred in Monroe County in order for the records to be on file in the Monroe County Health Department.

To obtain a certified copy of a birth or death certificate by mail, forward to our main office:

  1. a completed application form - available from our office

  2. or a written request with your signature stating the name and date of birth / death of the individual on the certificate is also acceptable

  3. check or money order to the Monroe County Health Department - $20.00

  4. Self Addressed stamped business envelope

     Orders will be processed immediately upon receipt.

To obtain a certified copy of a birth or death certificate in person at our main office please bring:

  1.  a completed application

  2.  check, money order or cash

       Orders will be processed as you wait.

Certified Certificate Fee   $20.00

    If we cannot find the record that you are requesting, our staff can contact the Ohio Department of Health Vital Statistics in Columbus to see if they have a record of the document.  We can then fax a written request for the certificate(s). 

    Delayed birth and death certificated are documents that were filed some time after the actual event.  Delayed birth and death certificates may be requested from the Monroe County Probate Court in the Monroe County Court House.

    Any birth or death record from 1867to 1907 can be obtained at the Monroe County Court House, Probate Court at (740)472-1654.  If you need a birth or death record from another county in Ohio or West Virginia, our staff can give you a telephone number and address so that you can obtain the information from that county.

    Any person who is looking for birth or death records for family genealogy history is welcome to come to our office and review the birth and death rosters.     

   

    In order to request a certified birth or death certificate by mail, you must send a written request with $20.00 per certificate to the Monroe County Health Department.  We will send your certified copies back to you as soon as possible.
