The Monroe County Health Department Vital Statistics offers Birth and Death records from 1909 to date.
We now offer central issuance. If you were born in Ohio, we can issue you a certified birth record. Previously you were required to purchase your record from the county in which you were born.
Death records are only available from the counties in which the death took place.

To obtain a certified copy of a birth or death certificate by mail, forward to our main office:

Or a written request with your signature stating the name and date of birth / death of the individual on the certificate
Check or money order to the Monroe County Health Department - $25.00

Birth
records are now available to anyone who was born in the state of Ohio.
Previously we were only able to obtain records that were filed in Monroe
County. Call us to see if your birth record is available.

Delayed birth and death certificates are
documents that were filed some time after the actual event. Delayed birth
and death certificates may be requested from the Monroe County Probate Court in
the Monroe County Court House.
Any birth or death record from 1867 to 1907
can be obtained at the Monroe County Court House, Probate Court at
(740)472-1654. If you need a birth or death record from another county in
Ohio or West Virginia, our staff can give you a telephone number and address so
that you can obtain the information from that county.
Any person who is looking for birth or death
records for family genealogy history is welcome to come to our office and review
the birth and death rosters.
